The Kidney Health team at WesternU Health is dedicated to focusing on prevention. Meeting the goals for blood glucose and blood pressure control can prevent or at least slow the development and progression of kidney disease.<\/p>\n<p>If kidney disease goes without treatment, the disease will progress through the stages, ending in kidney failure. Once the kidneys have reached end-stage kidney disease, dialysis (a method of removing wastes from the blood) or a kidney transplant must be performed to survive.<\/p>\n<p>The Kidney Health team has implemented a team-based care model that includes a Nephrologist, Nurse Practitioner, Pharm.
